AS A COURTESY, our office will bill your insurance for appointment charges. Ultimately it is your responsibility to follow up with your insurance company to make sure that your account is paid in a timely fashion, including copays and deductibles (our contract with your insurance plan requires us to collect copays at the time of service). For unpaid balances older than 60 days a $5.00 billing fee will be charged each month. Please contact our billing clerk if there are problems regarding your account, and we will make every attempt to work out payment arrangements with you. However, if NO attempt is made to pay on your account, we will have no choice but to turn your account over to a collection agency. We accept cash, checks, Visa and Mastercard.
